Industrial Injury Claims
Requests for Employment Histories from HMRC, Special Section A - A New Procedure
With the help of a number of solicitors, Special Section A has successfully developed a new process to improve the existing procedure for compiling employment histories.
We have introduced a new form (PDF 52K) for solicitors to complete when requesting an employment history for a client. This form will bring the following benefits: -
- Easier and quicker tracing of HMRC employment records
- A faster response to requests for Employment histories
- Better customer service for solicitors and their clients.
If you are seeking information on employment history to make a claim for Industrial Compensation you will need to complete the form and post this to
H.M. Revenue & Customs
National Insurance Contributions Office

Special Section A must have the following before work can begin:
- The reason for the Employment History
- A signed form of consent to allow HMRC to release information (if the person is deceased a signature from next of kin or executor and a statement of the relationship to the deceased)
- National Insurance number (if known)
- The full name, address, date of birth and date of death (if applicable) of applicant.
Failure to provide any of the above information will result in a delay in the Employment History being provided.
An example of a completed form (PDF 58K) has been produced to assist you in completing the information.
Please start using the form immediately
When completing the form it is important that you give as much information as possible, based on your client’s recollection of employers within the period concerned. If your client cannot provide employment details, write “No information available” on the form. Please send the completed form to us with your usual covering letter and consent form.
Please circulate the form in your own office and in any other branches of your company that you feel need to see it. We suggest that you retain the form electronically so that you have a ready supply for future use.
